School Closure Information

Emergency Information Cards

At the beginning of the year your child will bring home an Emergency Information Form. Please complete and return the form, giving addresses and phone numbers where you can be reached in the event of your child's illness or injury at school.

Reliable alternative phone numbers are also needed in the event a parent cannot be reached. During the year, please notify your child's school if you have a change of address or change a work or home phone number.

Emergency Plan

Rain or shine our school buses run. However, each family should have a plan covering what to do when children arrive home early in an emergency situation.

What is the best route home if your child cannot be delivered to his/her normal bus stop? Where could they go for help? Who would care for your child until you arrive? Is there someone your child could call in case of an emergency?

Vision Screening

During the last week of September and all of October, vision screening will take place for all students grades K, 1, 2, 3, 4, 5, and 7.

Scoliosis Screening

During the last week of September and all of October, students in grades 5, 7, and 9 will be screened for scoliosis.

Illness and Medication at School

When your child is ill, please do not send him or her to school until (s)he is able to participate fully in all activities. In case of fever, the temperature should be back to normal at lease 24 hours before a child returns to school. If your child contracts a communicable disease, contact your doctor as well as the school nurse so that other parents may be notified.

Parents are encouraged to give medication outside of school hours. Medication can be given at school only with specific written instructions from a doctor and a signed parental permission form. If your child requires medication during school hours, please contact the school nurse.

Due to changes in the State requirements for administration of medicine at school we are no longer able to provide Tylenol, cough drops, or an other medications. All medications dispensed through the health room must be provided from home and have a physician authorization form completed by your health care provider for both prescription and over-the-counter medications.

Parents may now choose to send a one day only supply of either prescription or over-the-counter medication with their child. If you choose to have your child self-administer their medication, you must complete a parental permission form noting your child may take the particular medication independently. Both the physical authorization forms and the independent administration forms are available in all health rooms and can be faxed to your by request. We will send a copy of these forms home during the first week of school with the emergency treatment forms.

Due to the zero drug tolerance policy, your child must have a parental authorization form on file if they will be carrying their own one-day dose of medication. Self-carrying of medication is discouraged at the elementary level. Please refer any questions to your health room nurse or Sue Passalacqua, District Nurse, 360-618-6200.
